Radomski et al8 revised the diagnostic criteria for serotonin syndrome, which requires the addition of or increased dose of a serotonergic medication along with at least 4 major symptoms or 3 major plus 2 minor symptoms (Table 18,9). Also, the symptoms cannot be due to a preexisting psychiatric disorder; an infectious, toxic-metabolic, or endocrine cause; or recent treatment with a neuroleptic agent.Clinical suspicion of serotonin syndrome and recognition of presenting signs and symptoms are fundamental to diagnosis and treatment. Combined use of tramadol and mirtazapine, each possessing serotonergic effects, caused our patient& 8217;s serotonin syndrome. The incidence of serotonin syndrome may be underreported because of increasing and widespread SSRI use and underrecognition of its myriad of behavioral, autonomic, and neuromuscular symptoms. Serotonin syndrome also can masquerade as various endocrine, neuromuscular, and psychiatric conditions (Table 1). Morbidity and costly diagnostic procedures can be avoided by prompt recognition and appropriate intervention.Management of serotonin syndrome is aimed at discontinuation of the offending agent and providing
